Student Information and Art Release Form
Each piece of artwork submitted to the Burnet Cultural Arts Festival must be identified in the following manner:
On the front lower right, the name of the artist, grade, title of work, medium, school and teacher must be clearly printed on a label.
On the back, the contact information which lists the student’s name, the school, teacher name, teacher contact phone number and email addresses.
This form must be included in the portfolio in order for the artwork to be exhibited.
Deadline for submission is October 3rd, 2008.
Please note: Only one entry per student. Artwork not mounted or matted will not be displayed. Only 6 pieces per participating teacher are allowed. However, more than one art teacher per school may participate. Open to Kindergarten through 12th grade students.
Work from the same school may be submitted together in a portfolio, with the participating teachers or leaders listed, and contact information clearly marked on the front. This is to ensure the return of all items to the correct portfolio.

The organizers of the Burnet Cultural Arts Festival cannot be held responsible for any damage or theft of artwork submitted to the Art Exhibit. Artwork will be exhibited during the Festival only then made available for pick-up at the end of the festival. The teacher is responsible for the collection of the work and its return to the student.
